Aadhaar Form Guide

This official UIDAI form format is accepted at all Aadhaar Seva Kendras and Post Offices nationwide for both new enrolments and updating existing demographic records.

Required Proofs

  • POI: Proof of Identity (PAN, Passport, Voter ID) required for Name change.
  • POA: Proof of Address (Electricity Bill, Bank Passbook) required for Address update.
  • DOB: Birth Certificate or SSLC Book required for Date of Birth correction.

No Proof Required: Updating your Mobile Number, Email ID, or Biometrics (Photo/Fingerprints) does not require any additional document proof.
